Medication Reviews

If you take medication regularly, the doctor may ask you to book a Medication Review to ensure your medication remains appropriate and is at the optimal quantity and dosage for your condition. Depending on your medication, this review may be with the GP or with the Practice Pharmacist.

Structured Medication Reviews

The Practice Pharmacist may carry out an in depth review called a Structured Medication Review.

These are comprehensive reviews of a patient's medication, taking into consideration all aspects of their health. They are undertaken specifically for complex patients, for example:

  • those that live in care homes

  • are severely frail

  • take complex or many medications

  • take potentially addictive medications

  • take medications commonly associated with medication errors

These reviews are personalised and focus on safety, effectiveness and shared decision making.
